The smallest injectable treatment we do
A lip flip is a handful of units of neuromodulator placed very superficially just above the vermilion border of the upper lip. It is one of the shortest appointments on our menu and one of the easiest to get wrong, because the margin between the right dose and too much is genuinely narrow.
The muscle around your mouth, the orbicularis oris, is a ring. When it contracts, it purses and curls the lips inward. In some people it is strong enough that the upper lip almost vanishes when they smile. Relaxing a small portion of it lets the lip sit and roll outward, showing more of the pink that was already there.
Who tends to love it
People whose upper lip rolls under when they smile. People who show more gum than they would like. People who want to look like themselves on a slightly better day and are actively uncomfortable with the idea of filler. And people who already have lip filler but find their upper lip still disappears in photographs, because a flip changes movement in a way that a static gel cannot.
Who should skip it
If your lip is genuinely thin at rest, a flip will not fix that, because you cannot reveal volume that is not there. Filler is the honest answer, and if that is not something you want, the honest answer is to do nothing.
If your work depends on precise control of your mouth, think carefully. Singers, wind and brass players, and people who present or teach all day are the ones most likely to notice the temporary trade-off. We do not refuse to treat them, but we do make sure they have decided with the information rather than without it.
We do not treat during pregnancy or breastfeeding, in certain neuromuscular conditions, or over an active infection or cold sore in the area.
At the appointment
Your injector will watch your mouth at rest, smiling, speaking and pursing before deciding on placement. That observation is most of the skill. A few tiny, shallow injections are then placed along the upper lip. It is quick, and most people describe a brief sting rather than pain.
Because the dose is so small, a lip flip is frequently added to an appointment you were already having rather than booked on its own.
Afterwards
Little to see. Small raised points settle quickly and bruising is possible but uncommon at this dose. You will be asked to stay upright for a period and to avoid rubbing the area.
The effect is not immediate; it builds. Give it time before deciding it has not worked. If it turns out to be under-dosed for your muscle, that is corrected at review. If it is over-dosed, there is nothing to do but wait for it to wear off, which is precisely why we start conservatively.
If it is wrong for you, we say so before you book again. A nurse-practitioner-led clinic has a licence at stake. That is the whole reason we are careful - and the reason the honest answer sometimes is "not this, and not yet".

Every clinic lists who a treatment suits. This is the other half. If any of these is you, say so at the consultation and we will talk about what else might work, or about waiting.
- You want a fuller lip. Nothing is added, so no volume is created.
- Your upper lip is genuinely thin at rest. You cannot reveal volume that is not there.
- You sing, play a wind instrument or present all day. The trade-off is real.
- You are pregnant or breastfeeding. We will see you afterwards.
- You have a cold sore or infection at the injection site. We will postpone.
- You want more units for a bigger effect. An overdosed lip flip is uncomfortable and obvious.

We look at what is actually bothering you, ask what you have tried, and tell you honestly whether this is the right treatment. Costs nothing and commits you to nothing.

Plan
Your clinician maps the plan with you and agrees it before you commit to anything. The plan is signed off by our Medical Director before treatment begins.

Treatment
Your clinician carries out the plan exactly as it was agreed. You are told what is happening, where and why, before it starts - and the quoted price does not change on the chair.

Review
We see you again to check the result and adjust the plan for next time. The first appointment is the start of the plan, not the whole of it.

What is a lip flip?
A lip flip is a very small dose of botulinum toxin placed into the orbicularis oris muscle just above the upper lip border. Relaxing that muscle slightly stops it from curling the lip inward when you smile or speak, so the lip rolls out and shows more of its pink surface. No product goes into the lip itself.
Does a lip flip add volume?
No. Nothing is added, so no volume is created. What changes is how much of the lip you already have is visible. If you want a genuinely fuller lip, you need filler, and we will tell you that rather than letting you spend money on a treatment that cannot deliver what you are picturing.
Lip flip or lip filler, which should I have?
Choose a lip flip if your upper lip disappears when you smile, if you show a lot of gum, or if you want a subtle change with nothing added. Choose filler if the lip is genuinely thin, asymmetric or lacking structure. Some people have both, with the flip refining how the lip moves and the filler providing the volume.
Will it affect how I talk or drink?
It can, temporarily, and this is the honest downside people are not always told about. Because the muscle you use to purse your lips is being relaxed, some people notice difficulty with straws, whistling, certain sounds, or playing a wind instrument. It is dose dependent and it wears off. If your work or hobby depends on precise lip control, tell us before we treat you.
How many units does it take?
Very few compared with any other neuromodulator area, and the exact dose is decided at your consultation based on your muscle strength and how much movement you want to keep. This is an area where more is emphatically not better, and an overdosed lip flip is uncomfortable and obvious.
How long does it last?
It is temporary and it wears off gradually. Because the dose is small, the effect in this area tends not to be one of the longer-lasting neuromodulator treatments. We will give you a realistic expectation at consultation rather than quoting a number that suits the sale.
Can it fix a gummy smile?
It can help in some people, because the same principle applies to the muscles that raise the upper lip. Whether it will work for you depends on why you show gum when you smile, and there is more than one reason. We assess that specifically rather than assuming.
Who should not have a lip flip?
We do not treat anyone who is pregnant or breastfeeding, anyone with certain neuromuscular conditions, or anyone with an active infection at the injection site. We also think twice for professional singers, wind instrument players and anyone whose livelihood depends on fine lip movement, and we will explain the trade-off honestly before you decide.
