A double chin, clinically called submental fullness, is an accumulation of fat beneath the chin and along the upper neck that blurs the line between the jaw and the neck; it is influenced by genetics, jaw and neck anatomy, skin laxity and weight, which is why it can persist in people who are otherwise slim.

Every face is different. This page is the general picture. What is actually happening on your skin is something a nurse should look at up close, in clinic light, before anyone recommends a treatment.
Does it get worse? That depends on the concern and on you. It is a reason to start early, not a reason to panic - and we will tell you plainly where you are rather than where a treatment plan would be easiest to sell.
Plenty of people arrive assuming a double chin means they need to lose weight, and are frustrated because they already have. The anatomy explains why.
The area beneath the chin, the submental region, contains a discrete fat compartment. How much fat sits there is strongly influenced by genetics, in the same way that some people store fat on the hips and others on the abdomen. It does not necessarily respond proportionately to overall weight loss, and it is one of the more stubborn compartments for many people.
Underneath that fat sits the platysma, a broad thin sheet of muscle spanning the neck and lower face, which loosens with age. Above and behind it sits the mandible. And the skin over all of it loses elasticity over time.
So four separate things determine what your profile looks like: how much fat is in the compartment, how projected your chin and jaw are, how firm the platysma is, and how tight the skin is. A treatment that addresses one while ignoring the others is a partial answer.
This is worth its own paragraph because it explains a lot of frustration. The soft tissue under the chin needs something to drape over. A well-projected chin and a defined jaw create a clear angle between the face and the neck, and the same volume of submental fat looks far less prominent.
A smaller or more recessed chin, which is a common and entirely normal anatomical variation, gives that tissue less support. The result reads as a double chin even where the fat volume is modest. In some cases the most effective intervention is not reducing the fat at all but improving the underlying structure, and that will come up at your consultation.
The assessment separates three things.
Volume. Is there genuine fat in the submental compartment? It feels soft and full and creates a rounded contour beneath the jaw.
Laxity. Is the skin loose? Does it crepe or hang when the neck is extended, without corresponding fullness?
Structure. Where does your chin and jaw sit, and how much of the appearance is about the frame rather than the padding?
Getting this wrong has real consequences. Dissolving fat in someone whose main issue is laxity can leave the area emptier and looser than before. That is the outcome we are trying to avoid, and it is why we take our time on the assessment.
Fat dissolving injections target the fat itself. An injectable agent disrupts fat cell membranes in the treated area, and the contents are cleared through normal metabolic pathways. Because the cells are destroyed rather than shrunk, the reduction is intended to be lasting at stable weight. Expect swelling afterwards, which can be pronounced for a period, and plan around it. Your clinician will be specific with you about that before you book.

Medical weight loss is the right starting point where submental fullness is part of a broader picture. 04 · What actually happens
If you have been putting this off, this section is for you. A consultation costs you nothing and commits you to nothing - you are allowed to walk out having booked absolutely nothing.

In clinic light, up close, with your makeup off if you are wearing any. This is the part people dread and it takes about a minute.

05 · Straight answers
The questions people actually ask us on the phone, answered the way we answer them on the phone.
Because submental fullness is only partly about body weight. Genetics determine where you store fat and how much sits in the submental compartment. Jaw anatomy matters too: a smaller or more recessed chin gives the soft tissue under it less support, so the same amount of fat looks more prominent. Neck posture and skin laxity contribute as well. This is why some slim people have persistent submental fullness and some heavier people do not.
The injectable agent disrupts the membranes of fat cells in the treated area, and the released contents are then cleared through the body's normal metabolic and lymphatic processes. Because the fat cells themselves are destroyed rather than shrunk, the change is intended to be lasting in that area, provided your overall weight remains stable. Swelling after treatment is expected and can be significant for a period, which your clinician will explain in detail.
This is the question that determines the whole plan, and it is not always obvious from a photograph. Fat feels soft and full and creates a rounded contour. Laxity shows as skin that hangs or crepes without underlying fullness. Many people have both. Dissolving fat where the real problem is loose skin can leave the area looking emptier and looser, which is exactly why we assess rather than assume.

It is used for the skin-quality and mild laxity component of submental fullness rather than for fat volume. In many plans it is combined with fat reduction, because addressing volume without addressing the skin envelope can leave the result looking soft.
There is no good evidence that facial or neck exercises reduce submental fat, and devices marketed for the purpose are not supported by meaningful data. Fat distribution is not controlled by the muscles beneath it. We would rather tell you that plainly than let you spend months on something that will not work.
It often helps, and for some people it resolves the issue entirely. For others the submental compartment is stubbornly resistant, and the fullness remains even at a lower weight. Which route suits you depends on your assessment rather than on what you saw advertised.
Not quite. Submental fullness is primarily a volume problem, whereas what people call a turkey neck is usually loose, crepey skin with visible vertical banding of the platysma muscle. They frequently coexist. The treatments differ, and a plan built for one will not address the other, which is why the assessment separates them.
Often, yes, and it depends on how much fullness there is and which approach is used. We will not quote you a fixed number before assessing you, because that number depends entirely on your anatomy and your goal. What we will do is set a review point so we can decide together whether more is warranted rather than booking a course in advance.

Bring three things. Whatever you are currently putting on your face, a rough idea of when it is at its worst, and any questions you have written down. That is genuinely all the preparation we need.
Makeup is fine either way. If it is easier to arrive with it on, arrive with it on - there is a mirror and a cleanser in the room.
