Directorship is a professional relationship rather than a location. You are choosing the person who authors your mechanism, reviews it as your practice changes, and picks up when a client does not fit it.

One province, one regulator

A nurse injector’s authority in Ontario rests on two provincial things: registration with the College of Nurses of Ontario, and an authorizing mechanism issued by an authorized provider under the Nursing Act, 1991.

Neither is local. A directive authored by a Nurse Practitioner in Bradford is the same instrument whether the nurse using it works in Vaughan or in Thunder Bay. There is no regional licensing, no local registry, and no requirement that the author be nearby.

What distance does change is how the arrangement has to be written and what you should demand because of it. That is the honest subject of this page.

Where we are, and how far that is

Directorship: what changes with distance, and what does not

What does not change. The documents, the prescribing, the competence assessment at the start, and most case review. These run on phone, video and email for nurses thirty minutes away just as much as for nurses five hours away.

What does change. Three things get more important the further apart we are, and all three should be in writing.

The drafting has to anticipate more, because fewer questions can be settled informally. A directive for a remote practice needs to cover ordinary variation without requiring a call before every treatment, while still stopping clearly where consultation is genuinely required.

The route to reach the authorizing provider has to be explicit: how you make contact, what response time is realistic, and what you do with a client in the chair while you wait. Permission to call is not a route.

The complications plan has to name your local escalation, not ours. Which emergency department, what you send with the client, who you call on the way. For the filler emergencies with a short window, it has to be something you can begin without phoning anybody.

When we would tell you to look locally. If your practice regularly produces clients who fall outside a sensible directive, or you are early enough in injecting that you need somebody physically present while you build competence, then a local arrangement serves you better than we would. We would rather say that in the first conversation than take the work.

Regions we are asked about most

The Greater Toronto Area has its own pages, because the practical detail differs: Toronto for multi site and mobile practice, Vaughan where the drive is short enough to be in the room, Mississauga for nurses working under somebody else’s clinic arrangement, Brampton for the RPN scope question, Hamilton for how a remote first arrangement should be structured, and Barrie and Simcoe County for the one region where being nearby genuinely changes what is possible.

Ottawa and the east. Entirely remote in practice. Everything above about explicit routes and local escalation applies with the volume turned up.

London, Windsor and the southwest. The same, with the added point that a training trip is a genuine commitment and should be planned to do more than one thing.

Waterloo Region, Guelph and Wellington. Close enough that an occasional visit is possible, far enough that nothing should depend on it.

Durham, Peterborough and the Kawarthas. An easy enough drive that the first meeting is often in person.

Simcoe County, Muskoka and the near north. Our own region. Barrie is twenty minutes up the 400, and this is where in person case review is simplest.

Northern Ontario. Remote, planned carefully, and worth being blunt about. The arrangement works. The training trip is a real journey and should be built around an overnight stay and more than one course.

Training: in Bradford, and only in Bradford

Courses are taught in our own treatment rooms, with our own stock, our own emergency equipment and the clinic’s own clients. That is the reason to travel for them and the reason they cannot be exported to a function room somewhere else.

For nurses coming any distance, say so when you enquire. We will line up dates so one trip can carry a foundation course plus either the complications course or a day of shadowing, and tell you whether the timetable makes an overnight stay sensible. Newmarket and Barrie have hotels fifteen to twenty five minutes away.

Can you be my medical director if I am in Ottawa, Sudbury or Windsor?

Yes. Your registration with the College of Nurses of Ontario is provincial, and a Nurse Practitioner's authority to assess, diagnose, prescribe, issue direct orders and author directives is provincial too. What changes with distance is not the legality but the practicality, and the arrangement has to be written for how far apart we are.

At what distance does a remote arrangement stop making sense?

It is not about kilometres, it is about your practice. If you treat straightforward cases within a well drafted directive and consult occasionally by phone, distance barely matters. If you regularly see complex presentations, prior work from elsewhere, or clients who fall outside any sensible directive, you need somebody who can look at a person in the room, and that argues for a local arrangement whatever we would charge.

Do you travel to clinics?

Not as a routine part of an arrangement, and we would rather not build a fee around something that happens twice a year. What we do commit to is a stated route to reach the authorizing provider and a realistic response expectation, in writing.

Is training available anywhere other than Bradford?

No. Courses are taught in person in our own treatment rooms, with our own stock and our own clients, which is the reason they are worth travelling for and the reason they cannot be delivered in a hotel in another city.

I am in northern Ontario. Is a trip for training realistic?

It is a real commitment and worth planning as one. Nurses who come from Sudbury, North Bay or further usually stay overnight and combine a foundation course with either a complications day or shadowing, so a single journey does more than one thing. Tell us where you are travelling from and we will line the dates up rather than make you come twice.

Does the College require my director to be in my region?

No. The College of Nurses of Ontario expects the authorizing mechanism to be appropriate, current and actually followed. It does not impose a geographic requirement on who authors it. A directive that cannot be worked in practice fails the test regardless of where its author lives.
